1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We aim to make our customers living with dementia’s journey as easy and positive as possible. We ensure our practice is as dementia friendly as possible and provide a personal service with the flexibility to tailor this to the individual.

  • Dementia Awareness raising for staff

    Ensure all staff, attend a dementia awareness raising session

  • Appoint a Dementia Champion

    Dawn Dawson nominated to be a Dementia Champion to ensure the viewpoint of people with dementia is considered in all aspects of the organisation. Attend a minimum of one Hampshire Dementia Action Alliance meeting per year.

  • Facilitate understanding and sensitive handling of patients known to be living with dementia

    Whilst respecting client privacy, we will adapt our recording system to highlight relevant information provided.
