  • Dementia Awareness raising for staff

    Ensure all staff, attend a dementia awareness raising session

  • Engaging with pwd and their carers

    We identify our pwd living in the community and write to them to invite them in for a review with their doctor. We are encouraging the completion of the This Is me document with their carers and this is brought to the review

    We are screening more people for dementia with a view to early diagnosis

  • Better Care Planning for people with dementia

    We are developing better care plans for our patients with dementia in order toavoid unnecessary hospital admissions and improve communication with their family.
