Community pharmacists were known in the past as chemists. Like GPs, community pharmacists are part of the NHS family. Every day about 1.6 million people visit a pharmacy in England. Community pharmacies are situated in high street locations, in neighbourhood centres, in supermarkets and in the heart of the most deprived communities. Many are open long hours when other health care professionals are unavailable. There are several different types and sizes of community pharmacies ranging from the large chains with shops on every High Street or in edge of town supermarkets, to small individually owned pharmacies in small communities in the suburbs and often in deprived areas or rural settings. The traditional role of the community pharmacist as the healthcare professional who dispenses prescriptions written by doctors has changed. In recent years community pharmacists have been developing clinical services in addition to the traditional dispensing role to allow better integration and team working with the rest of the NHS.

1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Medicines2Home is an internet-based pharmacy specialising in deliveries of medication to the home of patients or their carers. We believe as a delivery based pharmacy we are in a unique position to identify the deterioration seen in dementia as well as offering support.
Many of the patients we deliver to are unable to leave the home due to various reasons such as frailty, no access to transport or medical conditions. These are often the patients that require the most support as signs of deterioration may be missed due to irregular contact with healthcare professionals.
Our staff have regular contact with patients or their carers either via the phone or face-to-face whilst delivering medications. Medicines2Home are therefore perfectly placed to offer advice, manage medication and liaise with other healthcare professionals on behalf of patients with dementia.
We believe we can play a vital role in helping patients and their carers get on with their lives. We firmly believe that a diagnosis of dementia should have a minimal impact on a person's quality of life.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
The biggest challenge any healthcare team faces against dementia is the issue of trust. Dementia can be very difficult to cope with for the patient and it takes a lot of trust for them to feel comfortable talking about the condition.
We believe we are already in a strong position as we have built a strong relationship with patients through our regular and personalised home delivery services.
The key challenge for us is making sure that all members of our team understand the nature of dementia as well as having an understanding of the medications used for this condition.
This knowledge will give our staff confidence in dealing with patients with dementia and as a result, give patients confidence in sharing their concerns.
Staff and team development
* Training sessions will be organised for the team in which we will learn about the typical signs of dementia, the medications used and an overall understanding of the condition.
* We will also undertake a training course in how to correctly approach dementia with carers and patients, including ways in which we can help.
* Individual reading of materials provided in dementia booklet.
Optimising medicines and patients support
* offer free deliveries directly to the home of patients throughout the day, including on Saturdays
* offer auto-ordering of repeat medications or a free phone call reminder
* arrange for medicine aids including dosette boxes
* provide a free telepohone number to allow patients andf carers the option to call directly and discuss any issues/ concerns
We understand that carers can often be the only point of contact, therefore we plan to keep them informed of any medication changes and discuss any issues with them regularly.
* publicise on our website and via our Twitter account about being a dementia friendly pharmacy
* introduce badges for delivery drivers in order to make them more approachable to patients who wish to talk about dementia
