Straight Road Doctors Surgery

We are based at Harold Hill and we have a patient list of 2500. We take care of two dementia nursing homes. There are two principals GPs, one nurse, Practice Manager and five members of clerical staff who provide an efficient service at the surgery

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Raise further awareness to Dementia in the local community by joining the Dementia Action Alliance.

Ensure patients have access to support and advice.

Leaflets and posters in surgery to make patients aware of signs/symptoms of dementia.

We have aimed to increase awareness by advising people to report for memory loss symptom and screening them for dementia if the symtoms indicate

Education of carer and family about dementia and the availability of the service


2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Training for members of staff and clinician is not ready and locally available

Constraint of time, constraint of space , no clear structure for support for people with dementia for their carers and families.


2. Actions

  • Appointment reminder tool

    We have a policy in the practice to put yellow flags on all patients with Dementia to remind them of their appointment.   We also offer these patients specific appointments after the surgery if the carers of the family members have multiple issues to discuss.

  • Doctors to provide Dementia Awareness Training for their Practice staff

    All Practice staff to have awareness and understanding of how people are affected by Dementia, this will help to ensure that people with Dementia and their carers receive good quality service when visiting the Practice.

  • Flexibility around appointment times

    Many carers find it difficult to get a person with dementia to an early morning appointment; therefore we look to offer flexibility around appointment times. Plus we could look to provide double appointments so they do not feel under pressure and we are able to explain things in more detail.
