1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We believe we can make a difference in our community by providing local training and information sessions.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Regardless of the leaflets available and the newspaper advertising that is placed, unfortunately the attendance at our organised events is poor.

2. Actions

  • Make our community dementia friendly

    We intend to attend events and be more involved with the local community and businesses. We will also work closely with schools and churches.

  • Continue to improve the lives of our residents who are living with dementia.

    We will continue to treat our residents as individuals, provide person centred care and promote choice and independence.
