1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We provide services to people with dementia and their carers and families that support them in their community and personal lives

We are improving our customer service through increasing staff awareness and understanding dementia

We respond to the needs of the individual and offer support

2. Actions

  • Dementia Awareness Raising

    We will hold a dementia friends awareness raising session for 6-10 members of our customer interfacing staff.

    We will improve the signage on our toilet doors and review the colour of our toilet seats to show contrast

    We offer our clients :

    The option to move into a quiet meeting room as soon as they arrive

    Wheelchair ramp access

  • Information Sharing and Empowerment

    We speak at each Carers Course run by the Berkshire Helathcare NHS Foundation Trust to provide information and literature on :

    Power of Attorney

    Court of Protection


    Driving and other sensitive issues

  • Information sharing and awareness raising

    We promote dementia friendly approaches in our internal and external communications

    We let other organisations know what we are doing so that they can become part of their local alliance

    We develop good practice within our wider chain ensuring that both our Newbury and Thatcham practices are included in our action plan
