How we provide this service The Council provide supported housing for people over the age of 60. This is also called Sheltered Housing. This service is supported by Housing & Property Service Unit, Supported Housing.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We will be completing an at least an annual support plan document with service users to highlight any support needs. This document is used as a needs and risk assessment tool, to prompt referrals

We will be Liaising with other agencies to help engage service users with support.

We will be Involving family and other agencies with the support plan

We will enrole staff on relevant training to support those suffering from Dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Ensuring a clear referral pathways to agencies or organisations that can provide direct assistance and support

    To continue completing support plans with the service user and involving other agencies and family where appropriate, thus enabling service users to stay in their own homes for longer

    To continue to support the service user to access services where there is evident support need

    To continue to facilitate appropriate activities in the Sheltered Housing Community Centres

    Supported Housing monthly meetings

    Relevant information can be forwarded to SHDC’s Communications Officer, who will distribute at their discretion

    Posters or leaflets can be distributed across SHDC’s 14 Sheltered Housing Community Centres

    Supported Housing staff can be updated regularly via team email

    Information can be forwarded at Health and Wellbeing Group Meetings
