1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We intend to

  • Provide information/education with regard to dementia.
  • Raise awareness
  • Refer and encourage by our Forum meetings and Outreach events

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

  • Some Community Groups and Faith groups do not recognise the challenges of dementia .
  • We do not believe Carers roles are fully appreciated
  • To encourage health professionals to provide early diagnosis as it has become clear that there are over 2000 people in this Borough undiagnosed.
