1. Action Plan

1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?  

  • A local multi- agency Dementia Strategy group is responsible for ensuring changes takes place to improve the experience of those diagnosed with dementia in our care.
  • A Dementia Action Plan has been created for the organisation.
  • Undertake a programme of education to ensure staff are appropriately trained in dementia care to enable an informed and effective workforce.
  • Introduce the Dementia CQUIN to increase the appropriate diagnosis of dementia and ensure the patient enters a supportive health pathway.
  • Improve the quality of patient care.
  • Work in collaboration with our healthcare partners to strive towards integrated healthcare services. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?  

  • Competing priorities – internally and externally.
  • Improving services and meeting the increasing demand.
  • Ensuring Knowing Me document is completed.
  • Creating positive working relationships with local care homes.
  • Addressing environmental challenges.
  • Increasing the knowledge of an increasing workforce.
  • Achieving a way of sharing access to a register that details patients in our locality that have a diagnosis of dementia.
