1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Raise further awareness to Dementia in the local community by joining the Dementia Action Alliance.

 Ensure patients have access to support and advice.

 Leaflets and posters in surgery to make patients aware of signs/symptoms of dementia.

 Information about the Dementia service provided by the surgery available on the practice website.

 Education of carer and family about dementia and the availability of the service

 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Training for members of staff and clinician is not ready and locally available

Constraint of time, constraint of space, no clear structure for support for people with dementia for their carers and families.

2. Actions

  • Reminders to be put on patient computer records. Appointment reminder tool.

    Checking status of all Dementia patients on register.

    All Practice staff to have awareness and understanding of how people are affected by Dementia, this will help to ensure that people with Dementia and their carers receive good quality service when visiting the Practice.
