1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We provide care to people in their own homes including those with Dementia thus promoting independent living and supporting their carers in the process. Our staff are trained to a high level in Dementia Awareness.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

General public awareness of dementia and its effect on the individual and their family. The public belief that if you have Dementia you must go into a residential care home which is clearly not always necessary.

  • Dementia Awareness raising for staff

    Ensure staff, especially all customer facing staff, attend a dementia awareness raising session

  • Dementia Services

    We provide home care services to people with Dementia promoting their
    independence and wellbeing. This could be through personal care, medication assistance, companionship, meal preparation and nutrition.

  • Appoint Dementia Champion

    We will appoint a member of staff as a dementia champion who will deliver dementia awareness to customer’s families, friends and relatives.

  • Appoint a Hampshire Dementia Ambassador

    We will appoint a member of staff to act as a Dementia Ambassador to raise awareness of dementia in the local community.
