1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

HW Parish Council’s main aim will be to make a positive difference to the quality of life for people living with dementia and their carers.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Reaching the wider community and overcoming the barriers of data protection.

  • Dementia Awareness raising for staff

    Ensure all staff, attend a dementia awareness raising session

    HWPC will consult with people living with dementia and their carers in order to assess their needs and improve quality of life for them.
