1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Colten Care is a care home provider, with three homes in Hampshire providing dedicated dementia care. We provide essential care services to those in the community, living with dementia, who are unable to continue living at home. We provide a variety of dementia care services including day care, respite care, and permanent placement including nursing if required.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Given the nature of our organisation, we are well set-up to meet the needs of those people living with dementia. We employ a wide variety of staff from registered nurses, healthcare assistants, activities staff, social carers as well as all the other ancillary service staff. To provide a high quality service, effective and regular training in dementia care is essential for all staff to create a culture of well-being, which enables our residents to lead as full and active life as possible.
Dementia Awareness Raising
To raise awareness and understanding of dementia amongst the communities within and around our homes; including our staff, residents and their friends and relatives
All staff appointed to care for residents with dementia receive dementia awareness training before they start. Since 2014 all new staff will undertaken dementia awareness sessions as part of their induction. Within the three months of employment, they will then undertake our own 2.5 day dementia course developed with Bournemouth University.
Colten Care Dementia Ambassador
We have appointed a Dementia Ambassador within Colten Care who will be responsible for our developing our dementia care service such that it is aligned to the requirements of the local communities. This will involve liaison with national and local dementia charities, clinical service providers and relatives / carers
Consultation and Engagement with community
To co-ordinate and implement regular quality audits including relative and resident surveys plus regular relative meetings to ensure we’re meeting the needs of those people living with dementia. We can then identify additions or changes to our services that might be of benefit to the local comm
Consultation & Engagement
We are committed to providing support to national local dementia charities and organisations including Dementia Action Groups. This support ranges from fundraising to dementia consultation / advice and providing meeting facilities on request
