1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

By offering local services for those with dementia and their families in the form of Day Care, Dementia Clinics and support groups to help improve engagement with Dementia in the community

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Challenges will be accessing those in the local community and enabling those to utilise the services we offer
