1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Providing extra care schemes designed to support people to live independently who are living with Dementia. Our Court Manager service in our sheltered schemes provides great support for residents. We work in partnership with social services to deliver care and support to individuals living with dementia and ensure our customer support plans are person centred and outcome focused.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
The infrastructure of some of our schemes can sometimes result in H21 being unable to continue to support residents living with dementia. This can pose a risk to the resident (and other residents living within the scheme).
In relation to the domiciliary care services we provide, the commissioning of these services (ie time and task based) can sometimes cause difficulty in supporting our customers living with dementia.
Provide information sessions for residents living with sheltered and extra care schemes.
Enhancing Care Worker Knowledge
Provide information sessions for care staff during staff meetings
Research effect recent changes to extra care schemes have had on residents and their carers living with dementia
Influence design of new extra care scheme
What has worked well on improvements in existing extra care schemes to be replicated in new scheme.
