1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Our practice aims to provide personalised care for all patients with Dementia, ensuring an early accurate diagnosis with support for the patient and the family throughout their journey.

We have implemented a number of service improvements which ensure that patients with Dementia get longer appointments and that they are invited to attend the surgery for a thorough review of their health and care needs once a year.

We have made changes to the signage in the building and ensured that all staff are trainedto communicate sensitively with patients who have Dementia.

We are working on improving communication with patients and their carers so that we reduce the chances of missed appointments in primary and secondary care.

We have written to all patients who have Dementia and encouraged them to identify their main carer so that we can support them.

We are looking in to making greater use of the “ this is me” document to ensure better

continuity and personalised care for patients with Dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Technology – systems to ensure that carers are made aware of appointments

Communication – ensuring that all communication is clear and that reminders are sent where appropriate

Personalised care – in the setting of a busy general practice. We aim to ensure that patients who have Dementia get enough time with their practice nurse or GP.

Anticipatory care – ensuring that patients’ wishes are met from the time of diagnosis to the end.

2. Actions

  • Encourage early and accurate diagnosis in primary care, especially in the high risk groups

    Description: Our practice has signed up to the National Dementia DES and have ensured that all patients who are in the high risk groups are asked about their memory and offered a screening test if there is any concern. This is followed up by early and appropriate referral to a memory clinic offering expert advice and support.

  • Support for carers of patients with Dementia

    Description: All patients with Dementia have been invited to identify their main carer and we have offered them carer support and encouraged emergency care planning. They are also offered a regular heatlh check and annual flu vaccine.

  • 3. Title: Ensure that practice staff are able to identify and support patients who have Dementia and care for them with dignity and respect

    Description: Enabling the early identification of a patient with Dementia (an alert on their notes) helps to support them when they phone the surgery or arrive at the surgery for an appointment

  • Training for doctors to deliver on the action “ I will die free from pain, fear and with dignity, cared for by people who are trained and supported in high quality palliative care. “
