1. Action Plan
1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better
outcomes for people with dementia and their carers?
The Medway NHS FT is a 550 bedded District General Hospital in North Kent. We are planning to acquire Dartford and Gravesham NHS Trust later this year and are midway through the acquisition process.
The Trust provides all general medical and surgical services; it has an 85,000 attendance emergency Department, 5,000 birth obstetric unit with level 3 NICU, paediatric unit with a community paediatric service. The medical unit provides care of the older person specialty, cardiac, respiratory, gastroenterology, stroke care and diabetic and endocrine.
Medway NHS FT is dedicated to developing excellent pathways of care for patients affected by dementia. This strategy reflects the national Dementia Strategy, Living Well with Dementia (2009) and will assist the trust with improvements required to ensure first class dementia care is given to the patients of Medway Maritime Hospital.
We have developed a dementia strategy, based on the national strategy, the objectives of which are listed below in question3. The projects to improve the care of patients with dementia are high on the trusts agenda. We are working hard to achieve the national dementia CQUIN which has already shown great improvements in dementia assessment within the trust this year. We have excellent links with our local partners and commissioners and are fully supportive of any local dementia initiatives.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
The challenges are largely environmental currently but the monies from the strategic health authority allocated in 2012 will assist to improve this. Training is available and staff are working through this training which has proved to be very beneficial. Another challenge is accessing local community dementia schemes from the acute trust, however, the trust is working with local commissioners to resolve this.
