1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

1.  We deliver training to carers of people with dementia

We deliver dementia awareness training to all our staff and volunteers

We support people with dementia through our services including day services, carers respite, community and housing services

We support people with dementia to access personal budgets.

We advocate on behalf of people with demenia

We support people with dementia to self advocate

We are working with Age UK nationally to further develop dementia friendly services.

    Develop Dementia Friendly Services.

    Review our current services and develop more dementia friendly services.  This work will be supported by a small grant from Age UK.

    Black and Minority Ethnic older people and dementia.

    Ensuring our services are inclusive to the whole community.

    Transgender and dementia.

    Ensuring our services are inclusive to the whole community.

  • Dementia Friendly Services

    Audit all premises to assess whether they are dementia friendly.  Following audit - implement audit recommendations.

    Audit our current leaflets and amend to ensure they are dementia friendly.

    Ensure all staff and volunteers receive Dementia Friends training.

    Identify additional staff to complete Dementia Champions training.
