1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

At Park Care Meals we try to understand the need of each individual and can tailor our service to give people not only a choice of meals each day but also choice in how and when those meals are delivered. This is to enable people to live there own life in the comfort of there own home.

We also understand that we are more than a food delivery service, we are people who care and make daily contact with a friendly smile.

2.  What are the challenges to delivering these outcomes from the perspective of your organisation?

The main challenge to delivering these outcomes is getting the message out to people and there carers that the service is there for them and continually educating our staff.

2. Actions

  • Develop Closer links with social services

We continually strive to keep close links with local social services so people can be signposted to the service. We plan to make these links stronger with more regular contact.

  • Training of front line staff

We plan to have Dementia awareness training for all our delivery staff
