Tendring Dementia Action Alliance

1. Action Plan

Our  aim is  to achieve an outstanding service. We are aware of the impact Dementia is having on our society and want to deliver the best service we can , to help support people living with Dementia and their carers. To help us do this,  we would like patients to participate in the Patients Participation forum,  to help guide and steer our practice to address the issues they have.

We will ensure that all our staff have undergone the Dementia Friend sessions to increase their knowledge and awareness, we will embed this in by having it as part of any new staffs induction.

We have a dedicated member of staff who is the Dementia a Lead for the surgery and once able will have our own Dementia Champion.

We have a system in place that identifies people with Dementia and Carers and we en devour to prioritize appropriate  appointments for them.

Our Staff will always support a person with Dementia by greeting them in the waiting area and then accompanying them to the room they are going to and then again to the exit on leaving, we are working towards making all our signage Dementia friendly and having Dementia support information in our waiting area.

We are working with the Alzheimers Society to hold  Dementia Support clinics in the surgery for People affected by dementia

  • Dementia Friends

    We will ensure that all our staff have undertaken the Dementia Friends Sessions and will in bed this in to any new staffs induction process

    We will have a designated member of staff to take the lead on Dementia. When possible we will have a member of staff that is a trained Dementia Champion, until that time we have agreed with Age Concern that they will be our named Dementia Champion

    We will have a robust system in place that identifies people living with Dementia or carers and will endevour to prioritise suitable appointment times and duration.

    Our staff will support where necessary to the treatment rooms and to exit following appointents

    We will ensure that all our signage is Dementia Friendly

  • Information

    we will have an information board with services available in the waiting room specificly focusing on Dementia Services and Support.

  • Dementia Voice

    We will invite PLWD to participate in our Patients Participation Group, we will write to them prior to a meeting to invite them to join. We have a facility on our website for people to leave ideas, comments and feedback if unable to attend meetings

  • Alzheimers Clinic

    We will work with the Alzheimers Society to support them to hold Clinics with in our practice. G.Ps , Nurses, etc will be able to refer patients / carers for support and information
