East Lynne Medical Centre

We are a GP surgery commissioned by the NHS to provide primary care services to our practice population. We offer a full range of services including reviews on the management of long term conditions and lifestyle advice to improve the health and wellbeing of our patients. We also offer support to those patients in a caring role via our practice care adviser.

Tendring Dementia Action Alliance

1. Action Plan

We can roll out dementia training to all staff.  We can make our practice more dementia friendly by thinking about internal decoration and the colours used.  We will look at longer and more flexibility around appointments for patients with dementia and their carers.    We will have a dedicated notice board advertising clubs, groups and drop in cafes

2. Actions

  • Understanding more about Dementia

    We will roll out dementia friends information sessions for all our staff.  Our first session will take place on the 22nd January 2019 for administration staff and some clinicians.  We hope to have around 20 staff present

  • Enviroment

    To familiarise ourselves with the Alzheimer’s Society Checklist – “A Guide to Making General Practice Dementia Friendly”

     We will nominate a member of staff who will co-ordinate and implement the checklist in our surgery, as far as is practicable, and cascade new information to members of staff.

  • Peer Support

    Title: To arrange a regular drop in for people affected by dementia Description: To organise a local space for a regular drop in event for people living with dementia and their carers, working in partnership with community services and organisations such as the Alzheimer’s Society family navigators and other local voluntary services.
  • Dementia Information

    To be aware of support and services  available and display it on a dedicated Dementia Information board

  • Dementia Champion Training

    To identify a memeber/members of staff to complete the Dementia Champion Training so they can then deliver Dementia Friend sessions to all staff
