1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

To support all people affected by dementia in getting the support they need

We will have all our staff and volunteers signed up to take the Dementia friends sessions

We will have a data base of all organisations that we can refer people to 

We will promote other services within our hub and the greater community, with leaflets , posters and Age concerns newsletter

We will offer our premises on a pre arranged time schedule to other charities that are supporting people living with dementia as a venue to provide that support eg dementia surgeries 

We would like to start a friendship befriending group once a week 

We will make our hub dementia friendly with the appropriate signage 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?
