1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

At Oor Crusader and North Road  surgery we have a dedicated Doctor: Dr Jatoi , whose specialism is dementia, Our aim is to be as inclusive and dementia friendly as possible. We would like to be recognized as a Dementia Friendly accredited surgery Within the surgery we work in partnership with a GP Care Advisor and the mental Health team, both who have surgeries within our surgeries, going forward, Health in mind are potentially going to hold two clinics a week. We aim to have all staff dementia aware and all to have been on the Dementia Friend awareness session by the end of 2020 We arranged for the Alzheimers family Navigator to have clinics booked within the surgery, but then cov-id stopped them from happening. This is a potential possibility should the Alzheimers Navigator service offer this when they are back out in the community We have in place a system that alerts us to any - one who is living with or caring for someone with dementia and a policy in place that they will get priority with pre bookable appointments. We have a vulnerable person alert in place, this will alert us if there have been problems with medication, missed appointments etc and then procedures are put in place to support the problem. We assess and then offer where appropriate a referral to the chemist for blister packs. Our GP care advisors will visit and put support in place and or signpost to the relevant organisation. When the patient is called through to the waiting room, if they are unaccompanied the Doctor or receptionist will greet them and accompany them to the relevant room There is a quiet room that is made available if some one becomes agitated or distressed The signage is being replaced with recognized Dementia Friendly Signage and we hope to have this in place by the end of September 20202. We have the correct grab rails in the toilets, however the contrasting colored toilet seat we had in place has twice been stolen- we hope to replace this by the end of the year. We are working with all the care homes we have patients in to promote the Herbert Protocol and are compiling a spreadsheet of all those that are signed up to it. We also are promoting my care Choices.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

COVID 19 Room availability for outside organisations  Obtaining relevant information to display on posters - notice boards-all the relevant information is on our website , which is easy to use and dementia Friendly

    we will have all our staff become Dementia Friends, by undertaking the Dementia friend Awareness sessions by end of 2020
