1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Lambwood Heights, part of Oakland Care, is registered care provider with the CQC and we have a dedicated floor which provides high-level care to memory care residents.  This floor, known as Beech Crescent, is staffed by specifically trained, compassionate staff.  It offers state-of-the art facilities including a Tovertafel table projector which enables residents to enjoy a complete interactive game experience.  The memory care floor also has dining rooms with large balconies, a tea room and a separate lounge. With our person-centred care packages in place our Lifestyles team can tailor an activity for each person so they enjoying do what the like the most.  Gardening is very popular. Our memory care residents also enjoy full use of all our wonderful facilities on the ground floor – our Café, hairdressing salon, cinema and courtyard garden.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

There are not many particular challenges in terms of care offered, the home is established with all enhanced facilities to enable the correct type of care to be given.  Sometimes the care team might need to offer more encouragement and support to a resident who does not want to be engaged in any activity.
