1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

As a Practice, we are in a prime position to support and give information to people with Dementia and their families & carers.

 We already engage with the local Dementia Coordinator and will be expanding on this work to make sure we are doing everything we can.

 By raising awareness of the issues people face and by signing all of our staff up to Dementia Friends, we will be more able to identify those who may need extra support.

 By engaging with local services, we will be able to signpost to appropriate resources.

 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

General Practice is under increasing pressure on both time and resources.  It can be especially difficult to release staff and clinicians for training.

 

2. Actions

  • Appoint a key member of staff to be the main link re dementia

    We will appoint a Receptionist to work with the Practice Manager to act as the main link to the practice.  They will keep an overview of the action plan, provision of information and to liaise with the local Dementia Coordinator.

  • Include Dementia in Staff Induction programme

     

    We will make the Dementia Friends short video https://www.dementiafriends.org.uk/register-digital-friend a part of all new staff induction with a view to them doing a face to face session in their first year
