1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Kare Plus Romford is a key Home Care services provider within the local community that it serves. However, it also shapes the pathway for other branches around the UK. We seek to ensure that every individual is given the opportunity to live their life with dignity, respect and without discrimination. Social inclusion and community involvement will play a key part in reducing isolation.

We would also seek to take a proactive approach to the well-being of the local community that we serve. This will entail the right to early and accurate diagnosis where conditions and well-being is impacted. Proactive early diagnosis makes for better chances of recovery.

Raising the awareness of Dementia via enhanced training for all carers that work in the local community when delivering Home Care Services. This will require upskilling the knowledge base via innovation methods that in return ensure good understanding on how Dementia affects others in their day-today lives.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Tackle Isolation And Promote Social Inclusion

We pledge to reduce isolation by developing new initiatives that capture interests and hobbies for individuals and signpost local activities.

Going one step further we will look to offer monthly social activities within their own home, this could range from hobbies, puzzles, colouring/arts or chess for example.

Health Checks

We pledge to offer our local community Health Care Checks, which will identify early signs of illness and promote prompt diagnosis.

This will consist of temperature, oxygen levels and blood pressure, with any unusually high readings being referred to the GP.

Training Carers On Dementia Care

We pledge to enhance our training provided to all carers delivering Home Care Services.

This will entail a blended training approach, with online, in-class and VR ‘A Walk through Dementia’ experience as precured via Alzheimer’s Research UK.
