Luton Falls Team, Cambridgeshire Community NHS Trust

Luton Falls Team aims to prevent unnecessary hospital admissions due to falls, reduce risk of further falls and carry out falls prevention. Patient will receive a multifactorial falls risk assessment and rapid intervention, equipment provision and signposting. Service operates Monday- Friday 8am -8pm and Saturday/ Sunday 8am-6pm and patients can be seen same day, next day or within a week as required dependant on urgency. Will accept referrals for those living in Luton. Referrals can be made from health and social care professionals directly via system one via electronic referral or calling CCS SPA on 0333 405 3000 or via email CCS-TR.LutonFallsTeam@nhs.net

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Falls Assessment and signposting for those that have dementia but may be at risk of falls, sustained falls or have fear of falling in LutonSupporting falls promotion talks at our local dementia cafe to raise awareness of falls and falls prevention among those with dementia and their famiy and carers The Luton Falls Team becoming dementia friends to increase dementia awareness in the teamPromoting Luton’s dementia friendly community and encouraging others support at events and networking opportunities.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Limited time and resource as the Luton Falls Team is a small team of 7 staff.
