1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
We offer support to patients with dementia via the Practice and our Patient participation group.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Getting patients involved in our events and activities.

Replace signage within our Practice
We have asked our local DAA volunteer to attend the practice to review our signage. All Signage will be replaced and additional signage added to assist patients
.

Purchase Dementia Friendly Clocks
We will replace clocks within the Practice to Dementia Friendly Clocks that indicate time in large digital print, day, date, and morning or afternoon.

Dementia Champion In The Practice
We will nominate a specific member of staff to become a Demetria Champion. They will assist patients with Dementia and patients and relatives will have a direct contact at the Practice with additional knowledge of local services and support.

All staff will receive dementia Friends Training.

Try to get more patients involved in our Demetia Carers Lunches.
We will review our Dementia Register on a quarterly basis and ensure all carers are invited to the Carers Lunches.
