Improving post diagnostic pathways

The DAA Action Group on Diagnosis and Post Diagnostic Pathways is creating, for and with GPs, highly localised information prescriptions with knowledgeable patient and carer friendly phone line support. This is initially being piloted with six practices in a variety of urban and rural locations currently well or less well served, in association with DAA member the Guideposts Trust. The work is fine grained and highly GP participation focused, and as such will be set at a level the Alzheimer’s Society, RCGP and other member information programmes can make use of. Most importantly, this localised work will seek to make available the help and services provided by all DAA members and local DAAs.
