Living Well Pilot - Halton Dementia Delivery Group

Living Well Pilot – Halton Dementia Delivery Group (who also oversees the Halton DAA) undertook a 6 month pilot for memory, falls and social isolation/loneliness screening in the community, undertaken by non-clinical, community based staff ( ie Housing, Community Bridge builders, Age UK). Recommendation from the Dementia Delivery Group, on completion of the pilot, was that a robust screening training programme be devised and rolled out across internal and external community based partners. Discussions are currently underway with Halton’s Health Improvement Team to progress this
