1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisations role om delivering better outcomes for people with dementia and their carers?

Dementia has been identified as one of our strategic priorities. Our role in delivering better  outcomes for people with dementia and carers is to be a leader within the local health and social care economy, commissioning patient led services that support patients and their carers to live well with dementia. We do this by listening to our partners and being responsive to feedback. The CCG is also the contract lead for Nottinghamshire Health Care Trust on behalf of the other CCGs and can influence services at a wider level. We are working with the third sector and developing a vibrant relationship  to underpin the 'clinical' services being commissioned.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Newark and Sherwood has the largest aging population across Nottinghamshire which means that we need to plan now for the future. In the financially challenging landscape within which the NHS operates, we have to be innovative ini the way we design and commission services. In essence we have to do more with less.

Awareness amongst our residence is another key challenge and we are working with our partners to develop a plan to increase awareness so that patients with dementia and their carers are more supported in the wider community.
