1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

YDN aims to be a point of contact for anyone seeking information about working age dementia (WAD), aimed at the Nottinghamshire area.  YDN is an independent website and facebook page that has potential for collaboration, community, and signposting for carers, agencies and groups with similar interests.  The main aim and objective is to improve the lives of those living with dementia and their carers in the present time.  The incidence of diagnosed WAD in Notts is currently circa x250, although this is suspected to be an under estimate of the real figure which could be double that.  Information on WAD is scant due to the relative rarity of the condition and YDN seeks to be somewhere to come to for some empathetic understanding at the very least.  Also, to help raise awareness of the condition both locally and nationally where possible. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The challenges are for people to find the website and facebook page in the first place, and for the information within them to be current and relevant.  The organisation is currently just one person who is a current carer, so there is potential for overload.  Also, lack of information made available to communicate.  Currently there are no natural routes for information to be conveyed through.  It tends to rely on being in the right place at the right time and YDN actively seeking information, which is not always possible.

  • Sharing of information

    My website and facebook pages are central points for information to be sought from.  Opportunities can arise sporadically and haphazardly for the chance to share information in additional ways such as via local media, often Radio Nottingham when they are focusing pieces on dementia.

    2016 - Fourth Quarter Update

    I update my facebook page with articles or information that is current and relevant or interesting.  It is very easy to 'share' things that I spot on there.  The website is more static information updated when I know there to be changes, which depends when/if I hear about them.  Networking is a necessary part of keeping things as up to date as possible, which can be a challenge.  But I operate in a respond-to-need kind of way, and if more requests came in, I would respond accordingly.  More proactive is my Dementia Friends Information Sessions held several times a year as advertised on the Dementia Friends website.

    2013 - Third Quarter Update

    When I find interesting or relevant articles, I share these on the YDN Facebook page.  Also, add things to the YDN website, such as possible research projects to be involved in.  Plus advertising for events such as the Memory Walk.

  • Respond to contacts via the website and facebook page

     My only point of contact with the outside world is via the website ie to invite email contact, and facebook page to receive somments on status updates, or private messages.  Depending on what contact comes in through those, I will respond accordingly and aim to be helpful at all times.

    Whenever people contact me via email or facebook, I aim to respond in the quickest possible time.  This is usually within 24hrs.

    2013 - Third Quarter Update

    One person has contacted me through the website regarding some dissertatino research, and I responded with the knowledge that I had. I advertised her project on my Facebook and website pages.

  • Promotion of the website to newly diagnosed patients through WAD

     A few years ago I had contact with staff at WAD (Working Age Dementia).  My business cards would be shared where approriate.  Now I no longer have this contact so dissemination of information via this method is likely ceased.

    This method of contact has now changed so is no longer relevant currently.  Unless staff at WAD still pass on my contact details, I do not know at the  moment.

  • YDN representative become a Dementia Friend Champion

     To become a Dementia Friend Champion.

    2016 - Fourth Quarter Update

    I became a Champion a hwile ago but took time to start delivering sessions.  have delivered Dementia Friends Informaiton Sessions to members of my Boys' Brigade and Girls' Association, members of my church, and members of the general public.  My next session is also booked to take place in January with already 12 attendees signed up.

    2013 - Third Quarter Update

    I attended a Dementia Champion training session and now need to arrange some sessions to put this training into action and sign-up some Dementia Friends.  Work/carer/life balance has so far delayed this, plus needing the confidence by doing a bit of a starter session for my first one.

  • Network through the local DAA

     Attend DAA meetings quarterly if possible and have conversations with people whever possible to seek new ways to develops YDN and its effectiveness.

    I attend most meetings, except when they fall on my work days or a unique reason.  YDN is an activity that is not part of my paid work.
