1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

At S & W Independent Living, we are committed to provide a high standard of care and support to people who wish to remain in their own homes, safely and comfortably. It is our aim to do this in a manner that our service users remain as independent as possible whilst still maintaining their dignity and privacy

Our experienced team spend time with clients and families offering services round the clock such as; personal care, palliative care,domestic tasks, prompting and assistance with medication and mobility support. Assessing their support needs and discussing their choices and preferred routines, our staff promote independent living in the home.

  • Involve the community and our service users in social events, to raise awareness for Dementia

    Arrange a yearly coffee morning to raise awareness for dementia. This will build on 2017's successful event

  • Person centred care plans

    Ensure that when a service user has their person-centred care plan completed, it includes a ‘This is me’ document if the patient is living with Dementia. Ensure this is completed with service users friends and closest people to support health professionals.
