1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

by carrying out Home Fire safety visits and by providing education and advice we can make people living with dementia and their carers safer

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Finding out where they live , ensuring that they understand the message we are giving and if they do not that their carer can act sufficiently on their behalf and because the condition is progressive ensuring that we revisit at appropriate intervals.
