1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

To support the quality of life of people living with Dementia and memory loss and their carers and families.

We are passionate to get early diagnosis of dementia.

We have a GP with Specialist Interest in Dementia who offers over and above support to our patients who have Dementia - their Carers and their families.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Identifying sufficient time for staff training.

Keeping up-to-date with changes including publications and patient information.

Encouraging patients/carers to engage with this service.

  • Dementia Support Group

    We are going to help set-up a dementia support group within the Practice - it has been identified that there are very few of these on offer in our community.
