1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

As a GP Practice we have excellent local knowledge of our surrounds, but also of our community. We feel as a Practice we are an excellent local source for members of the public to receive information. We feel we can interact and offer support and guidance to patients and carers affected by dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Having patients engage with the service. Also carers/relatives understanding and asking for help and information.

2. Actions

  • Display up to date information and literature in an easy and accessible manner

    Continue to provide leaflets and posters in the waiting area. Also we will update our website with relevant links and information.

  • Secure permission for carers to recieve copies of letters, appointments and reminders and to be involved at all stages of the patients journey

    Continue to update the medical system with alerts of carers details etc. Have the forms and information accessible to all patients.
