1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Five Towns GP Surgery identify, refer and support patients with dementia and their family/carers. We give up to date literature and events within the surrounding community.

We ensure all staff are trained to be dementia friendly, and we support local organisations and events.

We consult with patient support groups and encourage them to drive the actions on our DAA plan.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We do not foresee any challenges to delivering these actions.

We would ensure the effectiveness of these actions by periodical review. We will add the topic to the agenda of patient support groups, and consult with patients with dementia and their carers if they have any additional needs.
