WLCT Community Wellbeing Team serves the community of Cannock Chase encouraging every one to be active, creative and healthy. We work on a variety of projects and initiatives to help to improve the health profile of Cannock Chase through art and sport.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The Community Wellbeing team runs community projects and initiatives within Cannock Chase; all of our work is community led and support the needs of the residents of the district. Our role is ensuring that our projects/initiatives are actively supporting people living with dementia and their carers to be active and creative with in the local community.

We want to provide opportunities and support people living with dementia and carers to participate in physical activity that suits their needs and are of their choosing. We are able to signpost people to creative opportunities and are able to provide creative expertise to provide quality sensory art work shops specifically geared to people living with dementia.

The Well Active website is highly accessible with its use of film and other creative media to showcase activities on offer in Cannock Chase and surrounding area. Through Well Active we able to support providers within the community to ensure their services are accessible for people living with dementia and their carers.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The knowledge and understanding of dementia within the community groups that we support.

Funding to provide tailored projects and initiatives to people living with dementia and their carers.

Raising awareness of our services to the health and social care sector in order to support more people living with dementia and their carers.

  • Dementia Friends Awareness Sessions

    All members of the Community Wellbeing team will be encouraged to attend a Dementia Friends awareness session.

    All member of the IHL wellbeing team have participated in a Dementia Friends Session

  • Projects and Initiatives

    All relevant projects and initiatives will be created with people living with dementia and their carers in mind, with a consultation as part of the development of the project/initiatives and ensuring that they accessible.

    Deliver bespoke project for people living with dementia - thinking active.

    This is in partnership with Staffs Uni to see how CST is affected by physical activity.

  • Well Active Website

    To increase the profile of the Well Active website to signpost and inform people living with dementia and their carers of the variety of activities and venues in the Cannock Chase district.

    Dementia Awareness information has been included on the Well Active Website.

    This was launched on 10th March 2017
