Share the care

Share the Care will offer a day support service Tuesday to Saturday for people with dementia and short term memory loss. We provide a person centred approach based on the interests and backgrounds of clients and will offer a range of stimulating activities for people to take part in. The service will work in partnership with the local community, providers and other community interest organisations to provide a wide range of opportunities and activities. Our vision is to promote engagement and participation within the local community gaining valuable feedback from service users, families and carers which will help us shape our service.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Each service user will have a support package specific to their individual needs in agreement with both the client, their carers and families promoting choice and control.  Our service will promote engagement in a wide range of activities both past and new that will reflect the changing needs of clients in creating new friendships reducing social isolation whilst enhancing quality of life.   We will offer respite and support for carers and families to have quality time for themselves with the reassurance that their loved one is safe and well looked after. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Accepting a loved one has dementia is difficult for all the family and everyone concerned - sometimes resulting in frustration, anxiety, grief and despair.   Validation therapy will underpin and be the backbone of our service centre ensuring that wherever the person is within their illness we will work with them in a time and place that is meaningful to them. This will help to reduce any anxieties.  Our centre has been designed and furnished to replicate that of a home environment to ensure that clients feel safe and secure for the time they spend with us

2. Actions

  • Raise public awareness of Dementia

    Share the Care will raise the profile of Dementia across the borough by linking in with local organisations and services and implementating a communication strategy that provides regular updates and information.

  • Highlight our Commitments

    Share the Care will create a website which provides relavant information and signposting.  The website is sharethecarewigan.co.uk.   We will also create both a Facebook page and twitter account to link not only with partners but individuals to share information and best practice.
