1. Action Plan

1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers? 

Barnsley Hospital is the key provider of emergency and acute services to the population of Barnsley and is pleased to declare its commitment to delivering better outcomes for patients with dementia. 

Using established partnership arrangements, the Acute Trust, continues to work closely with colleagues within NHS Barnsley and the Metropolitan Borough Council, in the delivery of local action plans intended to meet the objectives within the Dementia Strategy (2009). 

A local multi-agency Dementia Strategy Steering Group is responsible for delivering the objective related to acute care – to improve the quality of hospital care for people with dementia. Key areas for development have been identified and form part of a three year work plan detailing how the experiences of patients with dementia and their carers will be improved in hospital. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation? 

The main challenges we face in delivering these outcomes are influenced by internal and external health care priorities and targets within an acute hospital setting, such as infection control, patient activity and volume. Also, to achieve a greater awareness in best practices in dementia care, through the promotion of a cultural change.

  • Dementia Care Mapping

    Following one successful ‘dementia care mapping’ course and a second one planned for March 2012, care mapping will be implemented across the organisation.

    Now completed third course in January 2013. There are 89 mappers trustwide.

  • Promote the Early Diagnosis of Dementia

    To promote the early diagnosis of dementia by developing pathways for dementia/delirium care.

  • Training and Development Needs

    To plan the training and development needs for clinical and non-clinical staff, at all levels across the organisation.

    BHNFT has a 12 month plan this includes working along University of Bradford in relation to Person- centred dementia care in acute hospitals this is in conjunction with the local training education board. Launch of intranet staff site for information around dementia. Relauch of butterfly scheme. Flyer twice a year on staff wage slips to raise awareness of dementia. Dementia awareness weeek 2014 planned activities across the Trust to promote dementia care. Activities boxes distributed Trust wide to promote usage across the hospital. Continue and developed working in partnership with Alzheimers Society to promote dementia awareness. Existing training in relation to safeguarding has a relevant section with regards to dementia. Development of Sugar Cube Dementia Cafe.

  • Improved Assessment Processes
