1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The simplest and most effective way for Mirfield Health Centre to make a difference, is to increase awareness of how our reception/ admin staff work with patients and their families/ carers.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The biggest challenge for Mirfield Health Centre is time. By Mirfield Health Centre working in partnership across NKCCG, with other health and social care providers in Kirklees, we hope to be able to make steady progress in improving what we do with all patients.

2. Actions
