Derbyshire Community Health Services

In DCHS we care for patients across a wide range of services, delivered from 133 sites including 13 community hospitals and 28 health centres across Derbyshire and in parts of Leicestershire, We care for people , both young and old in our community hospitals and in their homes. My specific area of work is in mental health and in particular people who have memory problems, the day hospitals I lead see people for assessment and treatment of their memory problems while they attend the day hospitals, Another area we are currently developing is OUTREACH work, we provide LIVING WELL with dementia education/health promotion programmes to people with memory problems and their carers in their local community.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

To provide dementia friends training to all clinical staff working with mental health patients in chesterfield and northeast derbyshire by end macrh 2016

To provide dementia education sessions to our partner agencies /colleagues

To support the local development of the chesterfield dementia action alliance in making chesterfield a demenita friendly town by promoting the work we are doing, and sharing good practice with other agencies

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The area of DCHS I work within specialises in the care of people with mental health problems , in particular memory problems (OPMH)

The age of our patients is commonly in the 65+ bracket (there are younger patients )

As a trust we therefore need to ensure their holistic needs are met, both physical and psychological.I aim to support/develop the provision of dementia education/updates  for staff working within the care of our patients.

    This is currently an unmet need within our service and we are scoping the need in order to look at best service provision to meet what is required

  • Pilot dementia training to our colleagues within Chesterfield and NE Derbyshire before end 2015

    We have been asked by many of our colleagues within DCHS and externally to share the skills we have in the care of our patients

    We have a programme under development that we intend to pilot within 6 months, this will be reviewed
