1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The Willows Medical Centre is a GP practice based in Nottingham with just under 4000 patients. As a GP Practice we are often the first port of call for patients, family members and carers.  We aim to continue to promote Dementia friendly Practice by making the practice more dementia friendly and welcoming. All staff have  awareness of early signs of dementia and the issues that affect individuals living with dementia and their carers. We have a nominated Dementia  champion within the practice.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

3.A lot of people are not very knowledgeable about Dementia so for the them to come on board might be  a challenge.

4.Also denial is a big issue. People developing the condition will be in denial and sometimes their relative , carers and children too.

  • Implement Robust Systems

    To have a robust system to improve dementia detection amongst registered patients and improve recording of dementia. Our practice reception area and website will reflect this plan.

  • Making the Practice More Dementia Friendly

    We aim to continue to promote a Dementia Friendly Practice by making the practice more dementia friendly and welcoming. All staff to have awareness of early signs of dementia and the issues that affect individuals living with dementia and their carers. We will nominate a Dementia champion within the practice.

  • Implement a register of dementia 'at risk' patients

    We will keep a register of Dementia at risk patients and review monthly.

  • Dementia Assessment

    Our clinicians will work to identify people with potential dementia from our practice list and offer a dementia assessment and give a diagnosis. Relevant Read codes will be used.

  • Referal Systems

    Where our doctors will need a second opinion we will offer a referral and if agreed we will  refer as soon as possible.

  • Joint Working

    Through our joint work with nursing and care homes we will have meetings and updates as to identify patients in their care who may have symptoms of dementia and similarly offer a dementia assessment.

    We will offer double appointment slot to allow more time for dementia consultations.

  • Promoting Awareness

    We will have a dedicated notice board within the practice to promote dementia awareness and identification and invite patients and relatives to call us is they suspect that their loved one might have dementia.

    We will ensure that we have up-to-date carer details and register and   offer supporting literature where appropriate. We will  also engage with other health colleagues and the community to promote dementia diagnosis.
