1. Action Plan

1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers? 

Nottingham North and East Clinical Commissioning Group is made up of 21 GP practices organised collectively to commission health services for a population of approximately 145,000 people based in Arnold, Burton Joyce, Calverton, Carlton, Colwick, Daybrook, Gedling, Giltbrook, Hucknall, Lowdham, Mapperley, Netherfield and Newthorpe. 

Our vision is ‘putting good health into practice’, which we will implement by: 

1. Improving the health of the community 

2. Securing the provision of safe, high quality services 

3. Achieving  financial balance and value for money 

We aim to commission appropriate models of care for older and vulnerable people with complex needs, ensuring all patients are treated with dignity and respect. Improving quality of life for people with dementia and their carers is a priority and as a local commissioning group we are well placed to work with our member GP practices, our patient population and local commissioning partners to improve the quality of new and existing services.

2. What are the challenges to delivering these outcomes from the perspective of your organisation? 

In the context of our ageing population and significant rises in the prevalence of dementia coupled with the current financial challenges in the NHS we need to drastically change the way in which we manage our existing resources to meet growing demand. As one of the many health problems of our ageing population we are challenged with maintaining dementia high on the agenda for our commissioning group and member practices and for the providers from whom we commission services. 

As a priority for our clinical commissioning group dementia is a focus for one of our GP clinical leads. This provides a strong advocate for the clinical benefits of diagnosing dementia and providing appropriate services for these patients. However we are still faced with the challenge of engaging all our GPs in the benefits of early diagnosis and delivering a consistent primary care response to memory problems.

  • Partnership Working

    - Work on health inequalities pilot with CCG / GBC to identify specific issues for carers of people  living with dementia

    - Work with NNE Local Partnership subgroups

    - Work with local community safety and vulnerable persons partnerships to promote dementia awareness.

    Scope options for MHIC and FLO to support patients living at home.

    - Provide training and support to General Practices in line with NICE guidelines and current best practice.

    - Promote National Dementia Strategy which aims to train 250,000 healthcare staff on basic dementia awareness by 2015.

    - NNE CCG to have named Dementia Champion- COMPLETED

  • Reduce inappropriate use of antipsychotic drugs

    Reduce inappropriate use of antipsychotic drugs through reviews of existing prescribing and implementation of new guidelines on use of antipsychotics for people with dementia.

    Deliver dementia friendly training to Nottinghamshire North and East Local Partnership Sub group and sign a dementia friendly charter declaring to support the concept of dementia friendly communities.
